Income documents are required prior to approval of your Operation School Bell Application

• If EMPLOYED, applicant must provide 3 MOST RECENT PAY STUBS, in DATE ORDER, for SELF and Each Wage Earner 18 years or older living in the household
• If UNEMPLOYED and receiving Unemployment, SSI / SSA / EDD , CalWorks, or Support Checks each wage earner 18 years of age or older must provide copy of government document
• If UNEMPLOYED, and not receiving State supplemental income, applicant must provide ONE (1) of the following documents for each wage earner 18 years of age or older
• • • W-2s for each wage earner IN THE HOUSEHOLD
    • OR PREVIOUS YEARS INCOME TAX RETURN for each wage earner with Signatures – Page 1 & 2 only
    • UNEMPLOYMENT INSURANCE documents
    • SUPPORT CHECK documentation

    Your School Liaison will contact you within 7 days after submittal of your application to obtain required income documents.  You will sign a printed copy of your application, with income and school enrollment verification, at your Meet & Greet appointment
